Percutaneous Vertebroplasty for Compression Fracture in Elderly Patients

Abstract
Objective: Vertebral compression fractures were usually treated conservatively, but might be required surgical treatment if there is no response to conservative treatment. In the case of the elderly, it was tend to performed percutaneous vertebroplasty because of difficulty of surgery under general anesthesia. Therefore, we had conducted a study to evaluate the effect of percutaneous vertebroplasty in the elderly.

Methods: A retrospective review was conducted of 89 patients treated by percutaneous vertebroplasty from October 2011 to December 2013. Male was 14 people and female was 75 people. The median age was 76 years old (65 to 88). The patients had symptomatic compression fractures and failed medical therapy. Immediate and long-term clinical outcomes and complications of the percutaneous vertebroplasty with PMMA were evaluated.

Results: 65 (73.0%) of the 89 patients reported pain improvement within the first 48 hours. 43 patients (48.3%) were evaluated as excellent, 22 patients (24.7%) as good, 17 patients (19.1%) as fair, and 7 patients (10.5%), as poor. 14 (36.8%) patients had cement extrusion into a disc, paravertebral vessels or epidural space and all of them had no significant systemic symptoms except back pain.

Conclusion: Percutaneuos vertebroplasty using PMMA provided significant pain relief in most of the patients with compression fractures. It is considered that percutaneuos vertebroplasty using PMMA is an alternative method of treating patients with compression fractures who did not respond to medical treatments in the elderly.
